Router/Version: Netgear XR500
File/Kernel: dd-wrt-webupgrade.bin (r49418) / Linux 4.9.319 #1050 SMP Mon Jul 4 04:12:10 +07 2022 armv7l
Previous/Reset: DD-WRT v3.0-r49392 std (06/29/22) / no
Mode/Status: AP, SFE, AP 5GHz Mixed 80MHz + AP 2.4GHz NG-Mixed 40MHz, DDNS (Dynu), 802.11 encapsulation offloading, ondemand CPU governor at night, USB Storage (guide by egc), lighttpd (using Entware’s php8), Cron / working
Issues/Errors: maybe?

Router was working fine for 2 days then it suddenly died: red power led, unable to ping, WiFi network disappeared. Power cycle didn't help. Log after power cycle "capture.txt" is attached.
I was able to recover to stock Netgear firmware XR500-V2.3.2.134 using TFTP method, then I proceeded to flash factory-to-ddwrt.img r49418, but the router boot looped.
After that, I recovered to stock Netgear FW again, then flashed factory-to-ddwrt.img r47618, but I also encountered boot loop. I used the steps from "Netgear R7800 and XR500 Install Guide v43.pdf". Previously, there was no issue following the steps in the guide, but now I can't flash to ddwrt anymore. Serial log "capture-tftp_2.txt" attached.
Now I'm back to stock Netgear FW again. Router is working fine in AP mode.

Any suggestion on how to flash ddwrt?

EDIT: Sorry I just realize my USB RS232 converter had some issue and the log was garbled. After I unplug/replug the USB port, the serial console returns clean. Should I go through the process and capture the log again?

Not looked at logs but i would check for bad blocks:
telnet access in DumaOS/os
connect with putty Telnet http://192.168.1.1/debug.htm

from cli:
dmesg | grep bad

"dmesg | grep bad"

if that returns nothing it should flash.

XR500 V2.3.2.134 has no telnet on /debug.htm
telnetenable2.exe doesn't work
I'll downgrade to 2.3.2.66

EDIT: ser2pl64.sys has just given my Windows a BSOD. After so many years not touching the USB-RS232 converter, I'm surprised this is still a thing.

Did you try TFTP flashing the factory-to-ddwrt.img file?

Pretty sure it's a valid option:

If you want to TFTP DDWRT use the .img files used for the first flash:

it works! Thanks for pointing out, I wasted my life for not reading again.
Router is running dd-wrt again.
I still don't know what's the cause. Is there any chance that my USB RS232 converter messed up with the router?
