At this moment existing DNS servers e.g. the ones from static DNS, or the WAN DNS or pushed from OpenVPN or WireGuard are added to the smart DNS servers (as plain DNS servers)
So even if you are using DoH or DoT the plain DNS servers are also used and that is of course not what you want.
You can meticulously remove all static DNS, Local DNS, ignore WAN DNS, not use WG DNS and disable the pushing of OpenVPN DNS servers but a simple switch which prevents the adding of DNS servers to the smartdns.conf is easier
So the switch will let you choose between using the already present DNS servers or use the DNS servers you specify the Additional SmartDNS options.
It is just as if you have specified no-resolv in DNSMasq
this switch is "must have"... especially when you have payed DNS services... all queries are doubled...

I don't have good suggestion but "available" is confusing for me...
"Only use servers specified in conf." is not good either don't know how to shorten it... my english is very bad...

Yes I agree that available is not the best way to describe already present DNS servers (from Static DNS, WAN DNS WireGuard and OpenVPN client)

use "SmartDNS servers only "... set to enable so only those in config will work..
of "do not fetch default DNS"... enable-- so, only smartdns will work..
i guess option 1 - "SmartDNS servers only " is easy to guess... _________________Atheros
