Mode/Status: 2.4ghz is not quite right
Reset: Yes

Issues/Errors: MAC filtering doesn't work on 2.4ghz. Rejects all connections if 'Permit only clients listed to access the wireless network' is enabled. Works fine on 5ghz. I can see everything is correct if I check the 'Wireless Client MAC List'

QOS is enabled.

Errors: none that I notice yet. I'm still learning DD-WRT & how to set it up the way I want to use the router. Wifi LED stays solid and doesn't blink. DD-WRT firmware upgrade webpage seems to fail during flashing most of the time.

Status: Flashed from D-Link 9.04 firmware using factory-to-ddwrt.bin filename at upgrade GUI page & Emergency Recovery Mode page. Can't remember if successful flashing model-firmware.bin filename at either of r38159 firmware or D-Link 9.04 upgrade GUI pages respectively, to this.

Reset: only to default settings during GUI flash but was not necessary.

Previous: D-Link DIR-615 h/w rev. I1 9.04 firmware &/or DD-WRT r38159.

Ghetto_Child, I can't imagine why you are flashing a five-year-old build unless you are looking at a VERY obsolete recommendation in the generally useless router database. Modern dd-wrt practice is to use recent builds, but with a look first at their new-build threads - in your case in the Atheros forum - to be sure there aren't horrible issues.

Right now dd-wrt development is heavily focused on bringing Wifi6 routers into the fold, so there are lots of changes and perhaps more issues than usual. FWIW, I've done really well with 58753 from December on a Dynalink router. If you prefer older, 57200 has been solid for me on several router models, but it's from July, so it is definitely getting a bit old. Rumor has it that the key issues with recent builds are likely to be fixed very soon, perhaps in the next release in fact, so waiting a bit may serve you well.

The one recent new-build-thread report on a D-Link DIR-615 router in particular was for build 58892 by user @tedm, FWIW.

But do shy away from ancient builds. Sooooo many security fixes have come along and have been addressed since the one you are using. You don't want to be on something that old.

My plan was to work my way up to the latest that can fit & function. I have a handful of routers to upgrade and I've never 3rd party flashed anything before. Since the 90's I've only done manufacturer bios & firmware.

I'm starting from least to most valuable. D-Link DIR-615 rev I1, Linksys WRT610N v1, NetGear WNR3500L v1; this one is bricked for some reason but it wasn't being flashed when it died from a power off cycle. I also have some ISP modem/Wi-Fi routers; Smart/RG, 2Wire, Thomson, Siemens.

I don't have TTL cables, or serial cables so it's best to start with lowest risk & easiest recovery. I read somewhere releases 40000 and later cannot work on small size flash ROMS. Post a link if you find proof otherwise.

I am sure you read wrong. The Linksys WRT54G series still gets updates (4MB Flash), although minor ones, and was the very first supported router. Could be there is a build bug for this particular router model. Dunno, but antique routers are still supported.
