The IGMP Proxy is configured with the igmpproxy.conf file.
It contains what interface is the source and to what interfaces to send Multicast to.
igmpproxy will also convert between IGMP v3 and v2.
The set top box may only support v2.
I was unable to find that file in DD-WRT, running igmprt will ask for a config file, which i create one, add some config info, but no changes in 5/10 freeze.
But even if igmprt pick´s up the config file, i´m not sure what config should use.
Most igmpproxy.info i find is for users replacing ISP router, not connectiog in Client/Bridge mode.

Might be worth trying to enable IGMP Snooping on the switch:
swconfig dev switch0 set igmp_snooping 1
swconfig dev switch0 set apply
You can try running these commands via Telnet/SSL first and see if it makes a difference.
If so, you can add it to your Startup commands.
running: swconfig dev switch0 set igmp_snooping 1
will return "unknown command igmp_snooping
