Netgear R7000 - Plex remote connections show as router IP

I'm running a Netgear R7000 using firmware DD-WRT v3.0-r37015M kongac (09/23/1Cool. On my Plex server any streaming that happens from outside my network shows up as coming from my router's IP and not the external IP. I posted on the Plex forum and they suggested I come and post here for more help. I am manually port forwarded for the port that Plex is listening to. I have no VPN setup either.
